When it comes to tackling sun age spots, you have to get deep if you want to be meaningful.

Tan in haste, repent at leisure. Pigmentation is now the number one skin complaint in dermatology clinics on both sides of the Atlantic, with sun damage in its many forms showing up in over 80% of patients who walk through the door. It's also, by most surveys, considered more ageing than sagging skin - which says something about how much weight we put on an even tone versus almost anything else happening to our skin.


And yet we're repeat offenders. We know the risks, we've had the sunscreen lecture a hundred times, and we still come back from every holiday with a few new freckles and the quiet dread of knowing some of them are staying. Age spots and pigmentation aren't a malfunction, either - they're proof the body did exactly what it was built to do: produce more melanin as a defence against UV. The frustrating part is that the defence mechanism itself becomes the cosmetic concern.

What sun damage actually does to skin

UV exposure works on skin in two separate ways, which is why sun damage rarely shows up as just one thing. First, direct sunlight speeds up melanin production, and in high concentrations - or after years of compounded exposure - that melanin clumps rather than distributing evenly, leaving behind the brown patches most people mean when they say "sun damage" or "age spots." Occasionally sun exposure also produces small red marks called cherry angiomas, clusters of blood vessels rather than pigment, which is a different mechanism entirely but gets lumped into the same complaint.


Second, and less visibly, UV exposure upregulates matrix metalloproteinases - enzymes that break down collagen and elastin - which is the mechanism behind sun-related fine lines, wrinkles, and loss of firmness. "Every dermatologist will tell you to stay out of the sun," is the common refrain, and the collagen-degradation pathway is a large part of why: pigmentation is the visible complaint, but the structural damage underneath is arguably the bigger long-term issue.

Why sun damage feels like such a big deal

It's worth naming why this particular skin concern carries so much weight, culturally. Pigmentation is asymmetric, it's on the face, and unlike fine lines - which most people accept as a marker of time passing - dark patches read as damage rather than age, which seems to be why people find them harder to feel neutral about. That's a large part of why the market for pigmentation correction is as saturated, and as saturated with overpromising, as it is.

A note for darker skin tones

Most of the options above carry a real, well-documented risk that's worth naming on its own rather than burying in each bullet: on darker skin (Fitzpatrick types IV-VI), heat- and injury-based treatments can trigger post-inflammatory hyperpigmentation - new, sometimes more stubborn dark marks that form as skin heals from the treatment itself. It's a paradox worth knowing before booking anything: several of the treatments aimed at existing pigmentation carry a real risk of creating more of it.


  • In-clinic pigment lasers (Nd:YAG, Fraxel): The heat-based, pigment-targeting mechanism that makes these effective is the same mechanism that risks triggering new pigmentation in melanin-rich skin. Q-switched Nd:YAG is generally considered one of the safer choices for darker tones when parameters are set correctly, but it still needs a practitioner experienced specifically with darker skin, not just laser treatment generally.
  • IPL: Not considered suitable for darker skin tones at all in most clinical guidance, due to a higher risk of burns and post-inflammatory pigmentation.
  • Chemical peels and microdermabrasion: Anything that removes or abrades the surface layer can trigger the same inflammatory response, particularly at higher peel strengths.
  • Cryotherapy: The freeze-thaw injury it relies on can itself cause new pigmentation or, less commonly, permanent lightening of the treated spot on darker skin.
  • Microneedling and punch excision/subcision (for scarring): Any procedure that deliberately injures the skin to trigger a healing response carries this same trade-off - it's treating one mark by risking another.


None of this means darker skin tones have no options - it means the right choice depends much more heavily on finding a practitioner experienced with treating pigmentation on melanin-rich skin specifically, and on lower, more conservative settings than might be used on lighter skin, often meaning more sessions at gentler strength rather than fewer aggressive ones.

Medical and in-clinic treatment options

Once pigmentation is established, treatment options generally fall into a few categories:


  • In-clinic lasers: Q-switched Nd:YAG and Fraxel are commonly considered leaders for pigment removal specifically. The mechanism is selective photothermolysis - an intense, focused beam is absorbed by the brown pigment, heats it rapidly, and breaks it apart so the body can clear it. This is different in kind from lower-intensity light therapies, since it relies on heat and pigment-selective absorption to work. Expect real pain, a week of visible healing (swelling, oozing, crusting), and a real bill - initial consultation plus each session commonly runs into four figures by the end of a course.
  • IPL (Intense Pulsed Light): A broader-spectrum light treatment, generally gentler than Nd:YAG or Fraxel, though results are more variable and it isn't considered suitable for darker skin tones due to a higher risk of post-inflammatory pigmentation.
  • Chemical peels: Exfoliate the outer skin layers to encourage faster cell turnover, which can fade superficial pigmentation over a course of treatments, though deeper pigment tends to need something that reaches further than a peel does.
  • Microdermabrasion: Physically exfoliates the surface layer; can soften the appearance of dark patches short-term but isn't a lasting fix on its own.
  • Cryotherapy: Freezing individual spots can lighten or remove isolated pigmentation, generally used on smaller, well-defined areas rather than broad sun damage.
  • Topical treatments: Prescription-strength skin-lightening creams (often containing hydroquinone or similar agents) work by limiting further melanin production rather than clearing existing pigment, which is why they tend to prevent things from worsening more than they reverse years of build-up. Retinoids, vitamin C, and niacinamide are genuine, well-evidenced skin brighteners worth having in a daily routine, though on their own they're unlikely to make much visible difference to established, years-old pigmentation.
  • Nutrition: a supporting role from within: Diet plays a smaller but genuine part in sun resilience. Lycopene, the carotenoid that gives tomatoes their colour, has been shown in published research to reduce UV-induced skin reddening (erythema) by around 40% after 10 weeks of daily dietary intake - a modest but real protective effect, working from the inside as an antioxidant rather than as a barrier the way sunscreen is. Where low-level laser therapy fits

Low-level laser therapy (LLLT) is a different category of technology from the pigment-targeting lasers above - it uses lower-intensity coherent laser light to interact with skin cells without the heat mechanism heat-based devices rely on. It's a genuinely different modality from LED-based light therapy, too, which uses incoherent light and works through a different pathway - the two get lumped together often, but they're not interchangeable, and evidence for one doesn't transfer to the other.


On the laser side specifically, a 2024 study in the Journal of Cosmetic Dermatology treated 30 women with moderate-to-severe melasma and wrinkles using a 660nm continuous-wave laser, and found statistically significant improvement in skin rejuvenation, with no adverse effects or re-pigmentation reported at follow-up. A smaller 2025 case report from the same research group used a similar red diode laser on two women with freckling and solar lentigines specifically, and reported a marked reduction in pigmentation intensity and lesion size over 12 sessions, with no recurrence at one year. Both are useful, genuine data points - though both come from the same small research group with modest sample sizes, so it's early evidence rather than settled consensus, and worth describing that way rather than as a proven treatment.
